As expected, it does basically play almost exactly like Afterschool Tag, only this time it's set in a hospital. The illustrator isn't named in the credits or the product's page, but I'm pretty sure it was drawn by the same guy too, or someone who replicated his style extremely well. Unfortunately, while the game is more forgiving with letting you retry each day without having to start over from day 1, the stages themselves are considerably harder, sometimes to a bullshit level where it feels like it's actually impossible to avoid getting fucked, literally. There were multiple times where while trying to escape or deal with one girl, I ran straight into another girl or two blocking my path.
For example, while being chased by your traditional chaser, I run into TV static, which summons Sadako, who gives me a few seconds to find a room to hide in before she rapes me. With the chaser still behind me, that means my only way is forward, but then the next two rooms are blocked by a 3rd girl who demands I give a very specific item else I get raped if I enter those doors. As if that wasn't bad enough, Sadako gives you so little time you might not even have enough to reach the 1st door while running at full sprint, let alone a 2nd room. This kind of thing happened multiple times, so unless you're a masochist beyond the bed, you may have a bad time during the later stages if you get bad RNG.
Items are hard to come by, and it wasn't immediately clear how you're supposed to use the money you can find lying around.
Dealing with the girls themselves can also be quite tricky. Instead of being able to find notes that give you hints, sometimes a certain non-aggressive enemy will walk up to you and say something. But it can be very easy to miss it if you move to another room or floor just as she shows up and the hint won't necessarily be for something you're having trouble with. I still have no idea how I'm supposed to deal with the invisible enemy.
There's some short segments where you roam around the hospital during the day where you can talk to some NPCs and do some side-quests that amount to go from Point A to Point B and pick up some item. As far as I can tell, it's only purpose is for a tiny bit of story and background on the enemies you face at night, only issue is you're not allowed to run during the day so it all just feels so damn slow. The animations are still top tier, and honestly they make giving the game a try worth it by themselves, specially if you're into femdom. I really only wish the game's difficulty was adjusted because it can be downright aggravating at times.
